Mine or petroleum site operator notification
Before operations begin at a mine or petroleum site, the mine or petroleum site holder must notify the Regulator of the operator.
To notify the Regulator of an operator, you need to submit a completed Appointment and notification of operator of a mine or petroleum site form (DOCX, 83.18 KB).

Who can be a mine or petroleum site operator?
This may be the mine or petroleum site holder themselves or another person conducting a business or undertaking (PCBU) appointed by the mine or petroleum site holder to operate the mine or petroleum site.
How do I appoint a mine or petroleum site operator?
To appoint a mine or petroleum site operator, you need to submit a completed Appointment and notification of operator of a mine or petroleum site form (DOCX, 83.18 KB). You will need to provide details of the operator you are appointing and they will need to make a declaration that they agree to the appointment.
What if a mine or petroleum site operator’s contact details change?
A mine or petroleum site operator must notify the Regulator of any change to the operator’s contact details by submitting a completed Change of contact details of operator form (DOCX, 65.6 KB). Notification must be given as soon as practicable (and no later than 28 days) after the change.
